Workers Compensation
Injured employees complete Form 6 at the time of the accident or as soon as possible and return to the School Board Office.
Principal/Supervisor should complete and sign the Employer's Report (Form 7). Original forms should be sent to the School Board Office.
Reimbursement
Complete for Travel Expenses only. Both sides of this form must be completed, approved and forwarded to Accounting at the School Board Office.
